In this role the Arizona C&S Healthy First Steps Program Community Health Worker (CHW) will assess and educate expectant families. CHW activities will focus on supporting a panel of rising risk pregnant members and families with complex medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. Telephonic as well as face to face interactions with members are structured around pregnancy and postpartum education and screening. Health coaching for behavior change and selfadvocacy are also key tools in this role.
This is a full time telecommute position with the ability to travel locally within the county up to 25% of the time. The employee must reside within a commutable distance of the Phoenix, AZ area and have access to reliable transportation that will enable travel to member and/or patient sites within a designated area.
This position is fulltime (40 hours/week) Monday to Friday. Employees are required to have flexibility to work any of our 8hour shift schedules during our normal business hours of 8:00am 5:00pm.

Primary Responsibilities:
- Engage members telephonically and or face to face to complete assessments and identify health and SDOH needs
- Conduct comprehensive member assessment that includes biopsychosocial, functional, and behavioral health needs, and develops a member’s plan of care
- Review available member health records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)
- Utilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment
- Identify when supplemental assessments are needed and conduct supplemental assessments
- Identify member service needs related to health concerns
- Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goals
- Identify member support systems available and incorporate into plan of care
- Review plan benefits and identify appropriate programs and services based on heath needs and benefits
- Collaborate with members to create solutions to overcome barriers to achieving healthcare goals
- Identify relevant community resources available based on member needs
- Refer and enroll members to appropriate programs and services
- Facilitate member choice of preferred maternity and family planning providers
- Follow up with members to monitor and document completion of plan activities following standard protocols
- Research memberrelated health information (e.g., medications, clinical records, policies, benefit information) using internal and external systems
- Document information (e.g., assessment information, member interactions, referrals, follow up plans) in relevant computer systems using appropriate applications (e.g., Excel, Word, Outlook, Office Communicator)
- Execute and meet performance goals within a virtual environment

Required Qualifications:
- High school education
- Computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications (e.g., Excel, Word, Outlook, Office Communicator)
- Access to highspeed internet from home
- Dedicated workspace from home
- Reside within a commutable distance of the Phoenix, Arizona area
- Access to reliable transportation that will enable you to travel to member and / or patient sites within a designated area
- Ability to travel locally within the county up to 25% of the time with mileage reimbursement

Preferred Qualifications:
- Bachelor’s degree in social work, licensed practical nurse (LPN), CNA/HHA, or HS graduate with 3+ years of experience working within the community health setting in a health care role
- Experience working with maternity members or in obstetrics
- Field Based experience
- Experience working in Managed Care
- Experience in Counseling or Behavioral Health
- Case management experience
- Knowledge of Arizona Medicaid populations
- Bilingual fluency in English and Spanish
